We know your time is precious, but we also want to cover all the information you'll need to make decisions about your treatment. Here are steps you can take to prepare for your first visit.
Fill out new patient forms & health history forms. We have these forms available online so you can print them out and fill them out at your convenience. Then just bring them with you to your first visit. This will save time and allow more one-on-one time with your doctor.
Review our financing options & insurance information. Bring any insurance cards you may have to your first visit. Take a look at the financing options we offer so you can be prepared to use whatever option is best for you. If you're unsure what your insurance covers, don't worry! We will help you check your policy.
Make a list of any current medications, any concerns, & any allergies. We will need to know your medical history before any procedures, especially if you will be getting sedation or anesthesia. Please bring a complete list of medications, allergies, and other medical issues that could affect your dental procedure.
We went into dentistry to help people, and we love doing it! What could be more rewarding than helping people save their teeth or get them out of pain? Dr. Toole followed in the footsteps of her father, who practiced dentistry for more than 30 years. She took it further, specializing in endodontic dentistry, and has been certified since 2009.

After graduating from the UA-Fayetteville, Dr. Townsend got her dental surgery degree in Tennessee. After a year of practice, she decided endodontic surgery was her passion, and she received her certification in St. Louis.
